Apple Lossless Audio CODEC (ALAC), has launched a ISight Camera Replacement Program for iPhone 6 Plus. In the new support page created expressly for this replacement program, the company points out that a small percentage of iPhone 6 Plus devices may have an iSight rear camera with a component that could fail and cause photos to look blurry.
Is my iPhone 6 Plus eligible for replacement?

Apple has determined that, in a small percentage of devices 6 iPhone Plus, the iSight camera has a component that can fail and cause your photos to look blurry. The affected units fall into a limited serial number range and were primarily sold between September 2014 and January 2015.

From iPhone Hacks and based on this discussion thread on Apple support, it seems that few users of the 6 iPhone Plus They were affected by the problem, however, it prevents the camera from focusing properly so the photos are blurred, as we can see in the following image obtained from said forum.
At the moment it seems that the iPhone 6 has not been affected so the culprit of this problem could be the stabilization sensor that integrates the 6 iPhone Plus and that it is only available in this larger model.
To find out if your 6 iPhone Plus is eligible for this new replacement program, you must visit the replacement program support page and enter the serial number of your device that you can check in Settings → General → Information
Apple recommends that affected users back up their device to iTunes or iCloud to prepare their 6 iPhone Plus for the replacement process; It also notes that any other damage, such as a broken screen that makes it difficult to replace the camera, will need to be resolved prior to service.
The replacement program will cover affected iSight cameras on iPhone 6 Plus for three years after the device was first retailed.
NOTE: - Even if you haven't noticed anything strange in your photos, check if your iPhone is eligible for this program since, if so, it could still be affected in the future.
